Please note that once you have completed the Register for Online Services form (including reading & ticking the Terms and Conditions at the bottom of the form) in order to complete the process you will need to either bring in a photo ID to the surgery within four weeks, or email us a copy of a photo ID to surgeryreception@nhs.net within four weeks.
