Activity4Health Weight Management Programme: West Berkshire
The Activity4Health Programme in West Berkshire includes a free 12 week weight management programme for eligible patients who are referred in by their healthcare professional.
This is a service delivered jointly by Legacy Leisure, under the Activity4Health programme, and WW (formerly Weight Watchers). The combined delivery allows referred patients to access a holistic weight management service with a focus on behaviour and lifestyle change, diet and nutrition, and physical activity.
Referrals are to be made by an appropriate Healthcare Professional, to the leisure centre, via the referral form which can be found on the DXS.
The eligibility criteria are as follow:
– Aged 16+ years with a BMI ≥ 30 (BMI ≥ 28 Black/Minority Ethnic origin or those with long-term conditions);
– A resident of West Berkshire or a non-resident registered to a West Berkshire GP;
– Those on the Severe Mental Illness (SMI) register are eligible for the service so long as the healthcare professional making the referral considers the patient’s condition to be stable and that the content of the programme is appropriate for them.
