Patient Reference Group Registration

Welcome to The New Wokingham Road Surgery!

Our Patient Participation Group was formed in February 2010 and we are known as The Patients’ Group. We are a small, friendly group of patients who are all volunteers and our primary goal is to promote cooperation between the Surgery and patients, and the aims are for discussions of general interest and which are of benefit to all patients and staff. We meet every few months and our meetings are attended by two surgery representatives.

Please take a look at our Patient Participation Group notice board which is permanently displayed in the waiting room. It provides information on current activities as well as our latest newsletter.

The Surgery maintains a database of patients (The Patient Reference Group) that have volunteered to disclose their contact details so that they can receive, via email, Surgery related information, questionnaires and newsletters from time to time. The Surgery wishes to help the PPG to expand this to a wider range of patients and would like to ask you if you would please join the Patient Reference Group.

The consent asks you primarily to give your consent to both The Patient Participation Group and the Surgery to retain your contact details for the restricted purposes outlined above. The Surgery will maintain this database on secure servers and your email address will never be disclosed to other patients.

If you have any questions or concerns, we will be happy to raise them with the surgery. We also welcome new members to The Patients’ Group, so please contact me if you are interested and I would be more than happy to provide you with more information about our group.
