Sick Notes / Statement of Fitness for Work

General guidance for patients:

Doctors providing general medical services under the National Health Services Acts are required, under the Social Security Acts, to issue free of charge a Form Med 3 (Statement of Fitness for Work) to patients for whom they provide clinical care.

The Statement of Fitness for Work will usually be issued on the same day as the doctors assessment, but may be issued later if it would not have been considered reasonable to issue it on the day of assessment.

Doctors may also issue the Statement of Fitness for Work after consideration of a written report from another doctor or registered healthcare professional ( for example, nurses, occupational therapists, physiotherapists), if they consider it appropriate.


Please Note:

  • A Statement of Fitness for Work is not required for periods of sickness absence lasting 7 calendar days or less.

  • For patients who are on long-term sick, we require three working days notice to renew their sick notes.

  • Only Drs can write Fit Notes (GPs or Hospital Drs)

  • Some companies have problems with repeated short-term sickness. They may ask for a note regardless. In this case we can provide a private sick note. The cost for this is £25. Your employer should reimburse you for this.

  • You should complete a self-certification form – speak to work, search online (

  • From day 8 onwards, you require a sick note.

  • If you are seen before day 8, but the Dr believes you will be off for longer you may be given a Fit Note.

  • The DWP (Department of work and pensions) states that if you have 2 jobs, you only get one Fit Note – it is your responsibility to copy it.

  • The DWP states that we can only issue duplicates for lost Fit Notes.

  • It is possible to give you a Fit Note that allows you to work but with limitations: Change in duties (e.g., light duties only, no travel etc); Workplace adaptations (work from home only); Phased return to work; Reduced hours

  • If your employer cannot accommodate these changes, then you are not fit for work

  • Once you have reached the end of your sick note, you are automatically fit for work.

  • If your employer wants an assessment that you are ‘fit to work’ at the end of your Fit Note, they need to arrange for their Occupational Health advisor to assess you.

  • GPs are not qualified to assess what adaptations or considerations need to be made on your return to work, even if your employer thinks they are

  • Unless you have been told otherwise by one of our Drs, you usually do not need a face-to-face appointment to extend/renew your sick note
